​Each athlete requires the following documents to be handed in to their Head Coach

prior to the start of the 2026-2027 season:

• Copy of Birth Certificate

• Wallet sized photo

• 2026-2027  Medical History Form

• 2026-2027  Physical Fitness Form

• 2026-2027  Participant & Parental Contract

• Parent & Spectator Code of Conduct

• $150 Volunteer check post-dated to 12/31/2026​
